Daventry Primary Care Network Multi Disciplinary Team

This is an exciting opportunity to join the newly formed Daventry Primary Care Network (DCPN) Multi Disciplinary Team (MDT). The newly created network sees the two outstanding CQC practices of Abbey House Medical Practice and Danetre Medical Practice join together to deliver a set of seven national service specifications.

Based in Daventry, the team will have a lively and vibrant working atmosphere. Our size and resources will also enable us to offer a range of attractive benefits as well as unparalleled education and development opportunities for staff at all levels. The DPCN prides itself on its investment in staff both in terms of developing career skills but also its commitment to staff health and wellbeing.

The team will be made up of:        

  • Advanced Nurse Practitioners
  • Clinical Pharmacists
  • Social Prescribing Link Workers
  • Advanced Practice Physiotherapists
  • Physical Associates
  • Advanced Paramedic Practitioners

As part of this new initiative the integrated community-based teams will also be developing and configuring their services around the DPCN enabling the team to provide improved services to patients.

Recruitment has now commenced to these new positions and the team are looking in the first instance for an enthusiastic, highly motivated and forward thinking individual for the positions of Advanced Nurse Practitioner and Paramedic Practitioner to be based at Danetre Medical Practice.

The Daventry Primary Care Network is a forward thinking newly formed Network comprising of the CQC rated outstanding practices of Abbey House Medical Practice and Danetre Medical Practice.

Based in Daventry, this is an exciting opportunity to develop the new role of Social Prescriber within this Network.  Working as part of the general practice team you will help us deliver a comprehensive social care service that meets the needs of our patients.  This will involve taking referrals, ensuring that you target groups and communities most in need and helping them to live a healthier, happier life.  This will include patients who have been discharged from hospital, helping them to live independently according to their wishes, and connecting patients to voluntary groups and services for emotional, practical or physical support.

Ideally you will have an understanding of the voluntary sector and want to have a positive impact on the lives of our patients; this is an opportunity to really be involved with and develop a social prescribing scheme in the local area.  This will be a rewarding role for the right person, who will be a positive and proactive team player, also able to work independently with confidence.

Plus annual bonus dependent upon performance
DIRECT REPORT: To GP Clinical Lead
DATE: August 2019

The post holder is an experienced nurse, who acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice. They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures. In order to work at this level, NMC requirements for advanced practice must be met.
